ICCAD | Confirmation Form - Changes Requested

NOTE: Changes to title and/or author list are NOT permitted. Should you request a change to either item, you must indicate as such below and complete the form. All changes or author additions are subject to approval and not guaranteed.

ONLY USE THIS FORM IF UPDATES TO THE INFORMATION ON THE CONFIRMATION FORM ARE NECESSARY.

Updates allowed at this time are:

  1. Corrections to the spelling of names or words in the title
  2. Speaker designation update

If you do not have updates to the information listed on the confirmation form, submit the Confirmation Form - No Changes to the ICCAD office.

NOTE: If you have requested changes that require additional approvals, the submitter will be contacted personally by the ICCAD Office if the change was not approved. The ICCAD Office cannot guarantee that requests will be approved.

If you have requested allowed changes (spelling corrections, speaker change, etc.), please note that the ICCAD Office will work diligently to update our records and that the change will become visible once the ICCAD conference program is published to the ICCAD website.
